To complain or make a request for information to the Alberta Ombudsman, use the secure form below. Before you fill out the form, we strongly encourage you to read the information provided on this website regarding what the Ombudsman can investigate. The "Send Form" button will send your form in a secure and confidential web connection. We will notify you that we received your complaint by telephone or letter. The Ombudsman will not reply by email.
You must complete all the required information and click "Send Form" at the bottom. If you don't, the information will be lost. Please note if you have scanned copies and/or electronic files of your relevant complaint documents you are able to attach them to the Complaint Details section 4(b) and 5(b) of the online form. The allowable files are doc., docx., xls., xlsx., txt., pdf., zip., jpg., jpeg., gig., bmp, png. To attach a file (maximum 2.5MB), select browse and then locate the desired file on your computer and double click to attach.
